As the current or former owner or lessee of a Chevrolet Corvette vehicle, your satisfaction with our product is very important to General Motors. General Motors has received reports that wheels on some model year 2015 to 2019 Chevrolet Corvette Z06 and 2017 to 2019 Corvette Grand Sport vehicles have cracked or bent. Cracked or bent wheels caused by impact damage or road hazards are not covered under the New Vehicle Limited Warranty provided by General Motors. However, General Motors notified you by mail that if you have paid for a repair or replacement of a cracked or bent wheel on your Chevrolet Corvette vehicle that was not due to impact damage or a road hazard, you may be eligible for reimbursement under the following program.

Who Is Included in the Limited Special Reimbursement Program?

This program applies to owners or lessees of model year 2015 to 2019 Corvette Z06 and 2017 to 2019 Corvette Grand Sport vehicles. To be eligible for reimbursement under the program, you must have “Proof of Payment” that you paid to repair or replace a cracked or bent wheel on your vehicle within an extended warranty period of 4 years/48,000 miles (whichever comes first) from the New Vehicle Limited Warranty start date for your vehicle. You must also provide supporting documentation that the cracked or bent wheel was not caused by impact damage or road hazard damage. The amount of reimbursement or other alternative remedy available under the program varies depending on whether the repair or replacement was performed by a GM authorized dealer, the type of supporting documentation provided, and when the repair or replacement occurred.

What Proof of Payment is Required to Participate in the Repair Reimbursement Program?

For purposes of the Repair Reimbursement Program, “Proof of Payment” includes service records, receipts, or invoices provided by GM, a GM-authorized dealer, or any other vehicle repair or servicing entity. To the extent that a service record does not explicitly state that the amount due was paid, canceled checks, payment card records, or other bank record will be accepted as Proof of Payment.

What Supporting Documentation is Required to Participate in the Repair Reimbursement Program?

Current and former owners or lessees may submit two different types of supporting documentation that a cracked or bent wheel was not caused by impact damage or road hazard damage.

1) service records, receipts or invoices that explicitly state that a wheel repair or replacement was performed for a bent or cracked wheel and there was no impact damage or road hazard damage (such as cracks, punctures, cuts, snags, and breaks resulting from a pothole impact, curb impact, or from other objects, or words to the same effect) (“Supporting Evidence”)

OR

2) a sworn statement under penalty of perjury by the owner or lessee attesting that the vehicle received a wheel repair or replacement for a bent or cracked wheel and that, to the best understanding, knowledge, and  belief of the owner or lessee, the wheel repair or replacement was performed for reasons other than damage caused by impact damage or road hazard damage (“Supporting Affidavit”).How Do I Get a Reimbursement?

What is the Extended Warranty Coverage and Repair Reimbursement Program?

The Extended Warranty Coverage and Repair Reimbursement Program includes the following terms, conditions, and reimbursement amounts, which apply to wheel repairs or replacements for bent or cracked wheels performed during the extended warranty period of 4 years/48,000 miles (whichever comes first) from the New Vehicle Limited Warranty start date of the vehicle:

If you paid to repair or replace a bent or cracked wheel on your model year 2015 to 2019 Chevrolet Corvette Z06 or 2017 to 2019 Corvette Grand Sport during the extended warranty period and you submit Proof of Payment, with Supporting Evidence from a GM authorized dealer, you will be reimbursed for 100% of your out-of-pocket payment(s) for such wheel repair or replacement whether or not such wheel repair or replacement was performed at a GM-authorized dealer. However, if the out-of-pocket payment(s) exceed the cost of repair or replacement of a base OEM Wheel, reimbursement will be limited to the cost of a repair or replacement of a base OEM Wheel.

If you do not have Supporting Evidence from a GM-authorized dealership, you may still be eligible to receive an alternative remedy under the Repair Reimbursement Program if you submit Proof of Payment and Supporting Evidence from an entity other than a GM-authorized dealer.

If you do not have Supporting Evidence from any entity but provide Proof of Payment and a Supporting Affidavit, you may also still be eligible to receive an alternative remedy under the Repair Reimbursement Program, depending on the time and mileage of your vehicle at the time of the wheel repair or replacement and whether the repair was performed at a GM authorized dealer. You will not, however, be eligible to receive anything under the Program where there are service records, receipts, or invoices provided by GM, a GM-authorized dealer, or any other vehicle repair or servicing entity that explicitly state that a wheel repair or replacement was performed for reasons such as damage caused by impact damage or road hazard damage such as cracks, punctures, cuts, snags, and breaks resulting from pothole impact, curb impact, or from other objects, or words to the same effect.

Diagnosis, repair, or replacement of a wheel for aesthetic reasons or conditions other than the condition described above is not covered under this special coverage program.

How Do I Get a Reimbursement?

If you paid for a wheel repair or replacement as described above, please complete the Reimbursement Request Form that was included with your letter, along with all required documents, including Proof of Payment and either Supporting Evidence or a Supporting Affidavit, and a signed General Release.
Your completed Reimbursement Request Form, Proof of Payment and either Supporting Evidence or a Supporting Affidavit, and a signed General Release may be submitted by mail to the following address:

You may also submit your Reimbursement Request Form, Proof of Payment and either Supporting Evidence or a Supporting Affidavit, and a signed General Release by email to claims@CorvetteWheelsLimitedReimbursementProgram.com or by clicking on the “File Your Claim” button on this page. Please note, you will be required to upload your supporting documentation with the Claim Submission so please have that information available before starting your online claim.

GM-authorized dealerships will not be able to reimburse you for these repairs or replacements or answer questions about this Extended Warranty Coverage and Repair Reimbursement Program. You can receive reimbursement only by submitting the Reimbursement Request form along with all required documents to Analytics Consulting LLC.

Please be advised that the General Release will become effective only if your claim qualifies and you accept the reimbursement or alternative remedy offered to you in accordance with these terms.

Additional Terms

The right to participate in the Limited Special Reimbursement Program is provided only to those owners or lessees who complete the Reimbursement Request Form and General Release. Customers may not assign their right to submit reimbursement claims, or to receive reimbursement, or any other rights granted by this Special Coverage to any third party, including but not limited to service contract providers, and this Special Coverage is not intended to and does not confer any third party beneficiary, subrogation or contribution rights, or any other rights to reimbursement, against GM or Analytics Consulting LLC, whether in law, equity or otherwise, on any third parties.
